Immunization Schedule and Disease & Vaccine Information

Vaccines For Children (VFC) Program

In order for a child to qualify for this program, he or she must be 18 years or younger and meet one of the following requirements:

• Medicaid-eligible
• American Indian/Alaskan Native
• Uninsured
• Underinsured (defined as a child with health insurance, but the benefit plan does not include routine immunizations)

If you need immunizations and have insurance or are over age 18, please check with your physician or local pharmacy.

VFC Appointments

Please call  319-741-6422  and have the following information available during the call:

• Child's social security number
• Previous immunization records

There is no administration charge.
